Responsibilities
The Executive Director is accountable for overseeing the management of multiple markets within a designated Region. Executive Director is responsible for building a strong team and performance-based culture within their Region and executing across key value levers that support growth and success across HarmonyCares.
Essential Duties and Responsibilities
- Responsible for the strategy, growth, and operational oversight of one or more Regions across the organization
- Partner closely with other members of the market leadership team and health plan partners to provide oversight and execution of financial, operational, and quality metrics
- Work cross-functionally and strategically with Finance, Business Development, Operations, Care Management, Product, Actuary, Medical Management, Implementation team, Talent, and the Executive Team to achieve shared success and impact
- Mindfully monitor the performance of each market and LOB, applying best practices and lessons learned to advance all market's performance year over year
- Manage multi-million-dollar (>$25M revenue) P&L for the region, ensuring the appropriate value levers are in place to drive positive patient and financial outcomes
- Ensure clinical teams within the market are adequately focused on the needs of patients, and that staffing appropriately supports the needs of the business while allowing clinicians to work to the top of license
- Serve as a mentor to management teams, appropriately advocating for their needs while simultaneously holding them accountable for key performance indicators
- Standardize operations, where beneficial, to leverage lessons learned across markets to benefit improvements across the full organization – participating and owning special projects to support transformation of the organization
- Develop and nurture relationships with local health plan leadership teams, including sales and marketing, to support care coordination initiatives and membership growth
Qualifications
REQUIRED Knowledge, Skills and Experience
- Bachelor’s Degree
- 10+ years of leadership with experience in healthcare operations
- Extensive P&L experience with demonstrated success operating on budget and understanding key performance indicators
- Executive presence and collaborative mindset with multiple stakeholders that influence Harmony Cares including internal and external boards, health plan members, and employees
- Solution-oriented mindset and work approach, with a willingness to contribute to many different types of tasks to get things done in a rapid-paced start-up environment, even if they fall outside your normal scope of duties
- Cultural ambassador that can communicate and engage all levels of employees, both clinical and non-clinical
- Ability to travel up to 50%
Preferred Knowledge, Skills and Experience
- Experience with value-based risk contracts under capitated arrangements
- Experience with Medicare Advantage health plan offerings and populations of patients with chronic conditions – with an ability to execute a vision of creating strong best-in-class payor/provider solutions

Responsibilities
The Medical Pod Leader will be the clinical leader responsible for the clinical performance of clinical pod team members – to include: Accurate diagnosis and documentation, quality, patient experience, affordability, patient safety, clinical team dynamics, and provider productivity. The pod leader will be tasked with developing and maintaining close working relationships with all clinical team members in the clinical pod. The pod leader will also function as the collaborating physician for associated pod advanced practice providers (APPs). This position will have a dyad partnership with the Market Manager and market office staff.
Essential Duties and Responsibilities
- Act as on-site supervising Physician for assigned APPs, and Physicians assigned to the POD (collaborating physician). Total number of collaborative Practice agreements should not exceed state regulations.
- Collaborative time will correlate to assigned APPs and Physicians
- Participate in at least 1 ride along with all collaborative APPs per quarter
- Provide a complex patient visit as needed for each APP under supervision
- Collaborate with clinical team members associated with pod (Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, Patient Care Coordinators, etc.)
- Develop and participate in APP clinical development (Evidenced based care, professionalism, etc.)
- Lead and participate in high-risk huddles and ensure all patients discussed have a plan of care documented and includes all care management team members
- Build team-based culture and work collaboratively with clinical team members
- Conducts 10 chart audits per month or per state regulations, whichever is higher
- Act as resource for clinical team members for patient care questions/concerns and participate in difficult conversations with patients/family members as requested by team
- Routinely conduct panel reviews with APPs
- Sign DME, death certificates, and other orders as needed on behalf of APP (as regulated by state)
- Frequently review Quality metrics, pod level performance and identify areas for improvement leading to superior Quality (HEDIS) performance
- Frequently review clinician capacity and productivity performance. Partner with Site Medical Directors to develop countermeasures and support their implementation when provides are off track
- Work collaboratively with Regional Medical Director (RMD) to identify opportunities for care improvement
- Assists with service inquiry, resolution of patient complaints and requests for Pod as needed
- Assist pod providers to achieve higher EMR efficiency
- Participate in patient care to assigned panel and other patients as needed (High risk or complex patients, Physician face to face requirements, cognitive assessments, etc.)
- Participate and lead clinical education regarding accurate diagnosis, documentation and management of chronic conditions
- Assist with compliance and HR resolutions
- Responsible for discipline actions and performance reviews for direct reports
- Additional duties as assigned by the Regional Medical Director as related to clinical pod performance and outcomes.

Responsibilities
The Collaborating Physician works with an APN (or more than one) who is performing in home and/or telehealth wellness assessments on patients’ designated by HarmonyCares Medical Group. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients. As a Collaborating Physician you will:
- Be available via telecommunication or in-person for consultation with APN to address patient assessment questions
- Regularly performs annual chart reviews
- Consults with APN ensure adherence to policies, procedures, state laws and quality
- Be accountable for outstanding customer service to all external and internal customers
Qualifications
- Active/unrestricted Medical Physician License
- Board certified/board eligible required
- Residency completion required
- 1 year experience in Healthcare management
- Experience with Risk Adjustment
- Must reside in Indiana

The Executive Operations Associate supports the CEO's office. This position will provide hands-on executive support for the entire executive leadership team while also leveraging technology, AI, and automation to help improve how the CEO’s office operates.
This position will be proactive in anticipating the CEO’s needs, managing competing priorities, keeping commitments, and ensuring the executive office runs efficiently as well as helping to oversee daily office operations, coordinate meetings, visitors, vendors and manage supplies needs in order to create a professional and well-organized experience for employees and executives alike.
