Job Description
About the Role:
Our Devoted Preventative Primary Care Provider roles, include Nurse Practitioners and Physician Assistants and will be considered for both roles. As a Devoted Preventative Primary Care Provider, you’ll be part of a mission-driven, interdisciplinary clinical team focused on improving the health, dignity, and quality of life of our members. You’ll meet patients in the first step of their primary care journey with Devoted Medical. You’ll conduct comprehensive clinical assessments, support chronic condition management, and emphasize preventative and proactive care. You’ll collaborate closely with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to deliver coordinated, member-centered care.
This role sits at the intersection of preventative care, primary care and complex care management — it is not a traditional primary care panel role, and it’s not a one-time Annual Wellness Visit role. You’ll develop a relationship with your members while working in a supportive, tech-enabled environment designed to optimize clinical time and connection.
What You’ll Do:
Conduct primarily telehealth video visits, with limited and occasional in-home visits to members in your local area.
Perform comprehensive assessments and provide evidence-based care focused on prevention, chronic condition support, medication optimization, and health maintenance.
Identify and address care gaps, coordinate with external and internal care team members, and contribute to individualized, whole-person care plans.
Utilize integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time.
Communicate with empathy and clarity, fostering trust and confidence with members and caregivers.
Required Skills & Experience:
Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
Active and clear NP/PA license
(must be willing to obtain additional licenses if necessary within the first 90 days — support and reimbursement provided)
Active BLS certification at time of hire.
Comfort with delivering care via video telehealth and using electronic documentation systems.
Preferred Experience:
Familiarity with managed care models, including STARS/HEDIS and identification of care gaps.
Experience performing comprehensive or preventive care visits with Medicare populations.
Experience supporting older adults and/or individuals with complex medical or social needs.

Salary range: $115,000 - $140,000 annually

A bit about this role:
Our Pharmacy Guide team helps members overcome barriers that make it difficult to stay on their medications and navigate their healthcare needs. Pharmacy Guides play a critical role in improving medication adherence by connecting members to the right programs, resources, and support at the right time.
This is a fast-paced, high-volume outreach role focused on building trust, solving problems, and supporting members throughout their healthcare journey. Most of your day will involve outbound calls and SMS, helping members understand their options, resolve issues, and access the care and support they need.
Successful Pharmacy Guides are empathetic, adaptable, proactive, and comfortable balancing member experience with performance expectations in a rapidly evolving environment.
Responsibilities and Impact will include:
Conduct outbound outreach via phone and SMS to educate and engage members in medication adherence and health support programs. Most of your day will be on the phone with our members.
Build trust quickly with members through empathetic, solutions-oriented conversations that identify barriers to care
Enroll eligible members into pharmacy, financial assistance, and care support programs that improve health outcomes
Coordinate with providers, pharmacies, and internal clinical teams to help resolve member issues and support continuity of care
Schedule medical appointments for high-risk members when appropriate
Utilize internal systems, AI-assisted tools, and knowledge resources to research member questions and provide accurate information
Document member interactions accurately, manage follow-up tasks, and adapt quickly to changing workflows and business needs
Required skills and experience:
2+ years of experience in a high-volume customer-facing, outreach, or support role
Strong verbal and written communication skills with the ability to build trust and explain complex information clearly and compassionately
Comfortable spending most of the day on outbound phone outreach in a fast-paced, metrics-driven environment
Ability to manage multiple systems, workflows, and follow-up tasks accurately while adapting to changing priorities
Demonstrated accountability, ownership, coachability, and comfort receiving performance-based feedback
Desired skills and experience:
Healthcare, pharmacy, Medicare Advantage, care coordination, patient outreach, or contact center experience
Remote work experience
Work Schedule:
Hours of Operation: Monday - Friday 8:00 AM - 8:00 PM ET
Schedules Available: Monday - Friday 10:30 AM - 7:00 PM ET AND/OR 11:30 AM - 8:00 PM ET
Salary: $22.00 - $25.00 / hour

How do we optimize every Devoted touchpoint with members to ensure that they are empowered to complete life-saving preventive screenings? Sitting at the unique intersection of technology, clinical excellence, and member experience, you will translate enterprise quality goals into scalable engagement strategies, tech-enabled workflows, and high-impact clinical interventions. You will report to the Senior Director of Preventive Health with high visibility across executive leadership.
