About The Role
We're looking for a fullstack software engineer to help define the future of AI-powered, human-led healthcare. This is a hybrid role based out of our New York City (Flatiron) office, with a Boston office opening soon.
You'll carry real ownership over our product, with the chance to shape our engineering org as it grows. You'll work cross-functionally with product, clinical, and commercial teams, and interface directly with prominent healthcare organizations nationwide. Our engineering work spans full-stack web development, data infrastructure, mobile development, and AI/MLOps.

About The Role
We're looking for a Senior Backend Engineer to help define the future of AI-powered, human-led healthcare. You'll sit on our Platform & Integrations team, owning the core product primitives the rest of the company builds on: external APIs, health data models, retrieval infrastructure, and enterprise integrations.
This role is central to scaling our platform: supporting enterprise healthcare clients, powering AI-driven clinical workflows, and keeping distributed systems correct and reliable. Much of this is greenfield, especially how we model and operationalize longitudinal health data from wearables and external systems.
What You'll Do
Design and evolve our external API and embedded product layer with long-term stability, disciplined versioning, and enterprise-grade craftsmanship
Evolve our agentic retrieval harness to support patient and clinical search across structured and unstructured data, with strict permission and audit guarantees
Build reusable platform primitives that let external partners embed and extend our AI capabilities, so every integration adds lasting platform leverage
Contribute to core data models that unify structured health records, unstructured inputs, and high-frequency time-series signals

About The Role
Most healthcare still assumes you can walk into an office during business hours. That's left 100 million people without a primary care doctor, turning to Google with a billion medical questions a day instead of a person who can actually help. We're building something better: the new front door to primary care. AI-augmented, physician-supervised, and available 24/7.
We pair patients with in-house doctors through a messaging-first experience and give those doctors an AI clinician cockpit that makes them exponentially more effective. The result is a growing patient base and embedded partnerships with leading digital healthcare enterprises (OURA + more TBA).
This is a high-ownership role right at the center of that build. You'll shape the product experiences patients and physicians rely on every day.

What You’ll Do
Craft user interfaces for new paradigms of AI-enabled care that improve patient engagement and outcomes
Co-create and own product experiences across our patient app and physician-facing care delivery platform, keeping them polished, performant, and accessible
Shape frontend architecture, from design systems and state management to responsiveness, testing, and web performance
Partner with our ML research and backend teams to bring cutting-edge AI/LLM techniques into clinical workflows and turn them into real product experiences

About the role
At Counsel, we've built a product that patients and physicians love, on a fast-moving stack. As we scale, the line between "application code" and "infrastructure" keeps blurring and we need someone who lives right at that intersection.
We're looking for a Backend Infrastructure Engineer who's equal parts software engineer and infrastructure engineer. You won't be sitting in a silo writing Terraform all day, and you won't be shipping product features in React. You'll own the layer in between: the CI/CD pipelines, the IaC that provisions our AWS environment, the backend services that glue our systems together, and the developer tooling that keeps a fast-growing engineering team moving.
What You'll Do
Shape our infrastructure-as-code. Develop and maintain our cloud infrastructure using Terraform.
Build and improve CI/CD. Own our build, test, and deployment pipelines end to end. Your goal is to make deploying to production boring; fast, reliable, automatic.
Write real backend code. Contribute production backend services and core server-side systems. You write application code, rather than simply orchestrating it.
Build our core archetypes. Create production-grade patterns for new deployments, jobs, consumers, and workers. Think: an abstract base stream consumer or cron executor built for reuse across the team.
Make other engineers faster. Improve local dev workflows, cut build times, kill flaky infrastructure, and build the internal tools people actually want to use.
Strengthen security and compliance. Work closely with our security and compliance team to harden our infrastructure posture — secrets management, IAM policies, network isolation, and audit logging in a HIPAA-regulated environment.
Who You Are
Just as comfortable in an IDE writing application code as in a terminal managing cloud infrastructure, because backend and infra aren't separate disciplines to you.
Already scripting the fix by the third time you'd otherwise have to do something manually.
Understands how a request travels from a user's browser through a load balancer, into a container, to a database, and back, and can debug any layer along the way.
Reaches for the right tool over the trendy one and would rather ship something working today than architect something perfect next quarter.
Can explain infrastructure decisions to product engineers and translate product requirements into infrastructure changes.
What You'll Bring
3–5 years of professional experience in backend engineering, infrastructure engineering, or a mix of both.
Strong proficiency in at least one strongly-typed or compiled language (Go, Rust, Java, Typed Python, TypeScript, etc.).
Experience with AWS, GCP, or Azure cloud services.
Familiarity with containerized environments and Docker.
Bonus Points For
Hands-on experience writing and maintaining Terraform (or equivalent IaC tooling).
A track record building and maintaining CI/CD pipelines (GitHub Actions, CircleCI, or similar).
Kubernetes experience.
Experience with TypeScript/Node.js.
Experience working in a regulated environment (HIPAA, SOC 2, etc.).
