The primary location for this role is remote, and expected schedule requirements are Monday to Friday, 8:00am - 5:00pm and 10:30 am - 7:00pm.
What You’ll Do
As a Behavioral Health Care Manager (BHCM) with Imagine Pediatrics, you will work with the families of medically complex children providing case management for members enrolled in Imagine Pediatrics behavioral health program. You will work alongside pediatricians, nurses, care coordinators, and other healthcare professionals. Your primary responsibilities will include:
- Monitor high-risk pediatric patients (up to 21 years old), some recently discharged from the hospital, ensuring appropriate follow-up and adjusting care plans as needed.
- Conduct biopsychosocial assessments to address behavioral, social, emotional, and systemic needs of the patient and family.
- Create and evaluate the effectiveness of the patient/family’s care plan and modify based on families evolving needs and goal progression.
- Provide intervention that is consistent with the social/emotional/physical needs of patients and caregivers such as mental health crises, behavioral issues, and family conflict.
- Facilitate case management and support that requires clinical expertise in various systems with focus on helping patients and families negotiate the complexities involved with a mental health diagnosis and potentially co-occurring medical conditions.
- Resource validated external services requested by the family to meet behavioral and social needs such as social services agencies and behavioral specialists.
- Provides interventions in response to crisis to de-escalate and stabilize patient and family members
- Provides psychoeducation on the nature of mental health diagnosis and progression, the importance of treatment adherence, and related information as appropriate
- Collaborate with external care team members regularly including school systems, specialists, and child welfare agencies as needed.
- Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress
- Represent Imagine Pediatrics commendably to patients, families, providers, and community
- Performs other duties and assumes other responsibilities as assigned by manager

- Masters’ degree with major course work in social work or related field required
- LMSW required
- Minimum 3-5 years of post-graduate experience in health care social work/Case management in behavioral health required.
- Experience working with pediatric population and family systems required
- Proficiency in motivational interviewing practices and/or techniques; goal setting and intervention; assessment of needs
- Knowledge of social work including crisis prevention and intervention
- Experience with providing telehealth services
- Knowledge of MS Office Suite and ability to work in online platforms
- Strong knowledge of behavioral health principles and practices
- Proficient in trauma-informed care practices
- Strong knowledge of pediatric/adolescent mental health common signs and symptoms and able to identify difficulties with coping
- Role is remote with 10% travel necessary for training/education purposes
- Ability to work afternoons and evenings
- Bilingual Spanish strongly preferred

Family Resource Specialist

What You'll Do
As a Family Resource Specialist, you will provide direct support to patients and families in addressing social needs, working collaboratively with community partners and care team members to close gaps related to non-medical drivers of health. In this role, you will assist in building and strengthening community relationships, identifying and maintaining a robust
network of resources, and offering non-clinical support, guidance, and education to families navigating the unique challenges of caring for children with complex medical and behavioral health needs. In this position, you will:
- Identify resources and community support available to the family or caregivers
- Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services
- Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems and access needed services.
- Assist with accessing specific programs and supports to address psycho-social needs both within Imagine Pediatrics and in the larger community
- Advocate with community agencies for, and on behalf, of clients and families to decrease service gaps and remove barriers to services
- Connect with relevant internal staff and providers to assist with system navigation, including eligibility, pediatrics, behavioral health, public health, etc.
- Assist in development of an internal community resource library (but do not create clinical library materials)
- Provide linkage to community-based resources and school-based services
- Network with new and established community resources to increase knowledge of services
- Administratively document memoranda of understanding (MOU) for partnerships with outside community agencies
- Develop effective working relationships with interdisciplinary care teams
- Participate in ongoing scheduled consultations with an interdisciplinary team regarding administrative and non-clinical resources provided to family and caregivers
- Perform other duties as assigned

Community Resource Care Manager

While this role is primarily remote, candidate must be living in New Jersey. 10 % travel for offsite meetings, training, and educational purposes. 8:00am-5:00pm Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm.
What You’ll Do
As a Resource Specialist Care Manager, you will work with the families of medically complex children providing emotional support, care coordination, and resourcing support in navigating the healthcare system. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including physicians, RNs, care team assistants, mental health therapists, community health navigators, pharmacists, dietitians, and more. In this position, you will:
- Conduct thorough assessments, exploring the current medical, behavioral, and complex social needs.
- Identify barriers to care, such as food insecurity, unstable housing, transportation challenges, and insurance limitations.
- Collaborate with families and multidisciplinary teams to establish individualized care goals.
- Develop and maintain an integrated care plan addressing medical, behavioral, and social needs.
- Ensure timely follow-up on referrals, evaluations, and treatment recommendations.
- Offer education to families about navigating local resources and agencies.
- Provide emotional support and guidance when families are facing challenging situations.
- Advocate for resources and removal of barriers and connect families with available resources.
- Build and maintain relationships with community organizations that serve children with medical and behavioral health complexities.
- Stay informed on emerging programs and resources relevant to the population served.
- Represent the organization at community events, health fairs, and resource meetings.
- Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress.
- Perform other duties as assigned.
What You Bring & How You Qualify

Community Health Worker, Patient Engagement

What You’ll Do
Note: The primary location for this role is remote in Southern Missouri, and will entail frequent travel throughout the area.
As part of the Patient Engagement Team, this role supports patients in re-engaging with Imagine Pediatrics services, ensuring they receive appropriate support by meeting them where they are. The role partners with an interdisciplinary team to address patient needs and enhance re-engagement of patients who were previously enrolled but did not complete onboarding or have not actively utilized services, ensuring they are supported in reconnecting with Imagine Pediatrics care. You will:
- Serve as a community-based, patient-facing resource, building relationships with patients and caregivers through direct, in-person re-engagement in assigned geographic areas
- Provide hands-on support to patients and caregivers, including assisting with program re-enrollment, onboarding, and mobile app setup and utilization
- Preform telephonic outreach to caregivers of attributed patients to re-engage children in care, support seamless onboarding, and re-engage those who missed their initial onboarding visit or have not utilized services post-onboarding.
- Educate patients and caregivers on Imagine Pediatric services and available community resources, ensuring they understand how to access care and support
- Maintain and gather up-to-date knowledge of local community resources (e.g., social services, healthcare providers, SDOH supports) and connect families to appropriate services
- Conduct community events and facility rounding to engage eligible patients, strengthen relationships, and support ongoing program participation
- Partner with our Behavioral Health teams and local behavioral health providers to support safe patient discharge planning and ensure seamless connection to community-based resources and ongoing care.
- Partner with caregivers, families, and facility staff to identify and address barriers to engagement, and access to care and work with leadership on ensuring workflows exist to enable access to care
- Collaborate closely with the Patient Engagement Team (Outreach & Enrollment), including Outreach Specialists and Registered Nurses, to support patient re-engagement goals
- Support non-clinical care coordination efforts in the community, including assisting with:
- Connecting patients to primary care providers
- Facilitating pharmacy access and medication-related needs
- Providing basic discharge and follow-up support
- Addressing social determinants of health (SDOH) needs
- Removing barriers to care and service access
- Support outreach efforts through outbound engagement calls, as needed, to assist with enrollment, follow-up, and achievement of patient engagement metrics

Member Support Specialist (Remote, 8am - 5pm EST shift)

We are searching for a stellar Member Support Specialist who will support our Brightline members with signing up, scheduling, billing and any other administrative help they need. You will be joining a small, but mighty team at the cusp of an exciting growth phase, and be integral in helping us provide great support to every member who accesses our services.
Responsibilities:
Conducting first phone calls with new members to assess fit for Brightline services
Scheduling appointments for members
Ensuring member paperwork has been completed
Supporting Providers with a variety of administrative tasks
Answering all admin-related questions between sessions
Conducting eligibility checks, filing claims, and collecting member payment
Keeping our internal software tool for the Member Support team ultra-organized
Identifying ways to delight our members, ease the workday for our clinical team, and overall improve our operational processes as we scale
This position is fully remote with a 8am - 5pm EST schedule (Monday to Friday)

Licensing and Credentialing Manager
Brightline is redefining what pediatric behavioral health care looks like for families across the country. None of that reach matters if a provider cannot legally practice or a payer will not pay the claim. As Manager, Licensing & Credentialing, you own the machinery that keeps every provider licensed, enrolled, and revenue-ready across every state and every payer network Brightline works with. This is a remote role with no direct reports, and it is designed as a high-level individual contributor position.

Responsibilities
Licensing Management
Own the CertifyOS vendor relationship, including budget accountability and renewal negotiations.
Direct the day-to-day work CertifyOS performs on Brightline's behalf and hold the vendor accountable to defined service levels.
Oversee all provider licensing and license renewals across multiple states.
Ensure timely submission, tracking, and renewal of provider licenses.
Monitor vendor performance against established service level agreements (SLAs).
Partner with internal stakeholders to ensure providers are licensed before scheduled start dates.
Identify opportunities to improve licensing workflows through automation and process optimization.
Maintain accurate licensing records and ensure compliance with all state licensing board requirements.
Provider Credentialing
Manage all commercial, Medicaid, and managed Medicaid payer credentialing activities.
Personally oversee provider enrollment, re-credentialing, and payer applications from submission through approval.
Track credentialing status and proactively resolve enrollment delays.
Ensure providers are credentialed and billable prior to delivering patient care whenever possible.
Maintain complete and accurate credentialing documentation.
Monitor re-credentialing timelines to prevent credentialing lapses.
Payer Roster Management
Own payer roster management across all contracted health plans.
Ensure provider roster submissions are accurate, timely, and synchronized with payer systems.
Coordinate provider additions, terminations, demographic updates, tax ID changes, and location updates with contracted payers.
Perform routine payer roster audits to identify and correct discrepancies.
Partner with Revenue Cycle teams to resolve billing issues resulting from roster inaccuracies.
Establish standardized processes for roster maintenance as the organization scales.
Payer Relationship Management
Develop and maintain strong relationships with commercial and government payer credentialing representatives.
Lead recurring monthly payer operational meetings focused on credentialing escalations, enrollment turnaround times, provider roster accuracy, re-credentialing performance, and process improvement opportunities.
Serve as the organization's primary escalation point for payer credentialing issues.
Represent the licensing and credentialing function in cross-functional forums with Contracting, Revenue Cycle, Compliance, Clinical Operations, and Legal, speaking for the function's priorities and status rather than simply coordinating within it.
Partner closely with those teams to resolve payer related issues impacting provider enrollment.
Delegated Credentialing
Support the development of delegated credentialing programs with contracted payers.
Collaborate with Compliance and Quality teams to ensure delegated credentialing requirements are met.
Coordinate payer audits and maintain audit readiness.
Accountability and Scale
Own and report on the core metrics that define whether the function is working, including days to credential, license renewal on-time rate, and payer roster accuracy rate.
Define the processes, documentation, and workflows this function runs on today, built so they hold up as provider volume, state coverage, and payer relationships continue to scale.
Bring metric trends and process gaps to leadership proactively, rather than only when an issue has already caused a delay.
Requirements:
Bachelor's degree or equivalent combination of education and experience.
5+ years of provider licensing and payer credentialing experience.
Strong understanding of CAQH, NPPES, PECOS, Medicare enrollment, Medicaid enrollment, and commercial payer credentialing.
Excellent communication, negotiation, and relationship management skills.
Strong problem-solving and escalation management abilities.
Ability to manage multiple priorities while maintaining exceptional attention to detail.
Experience leading cross-functional initiatives that improve operational performance.

What You’ll Do
As Patient Engagement Associate with Imagine Pediatrics, you will be responsible for telephonic outreach to prospective members (parents, family members, caregivers) and helping them get enrolled and set up with Imagine’s app-based technology. Your responsibilities will include:
- Utilize telephonic outreach and engagement strategies to contact families and walk them through the process of successfully downloading the Imagine app and scheduling an enrollment visit.
- Effectively communicate Imagine services and overcome parent/family objections.

Care Coordinator (Non-Clinical)

The primary location of this position is remote (CST required) with an expected schedule of 3x12s (7:00am - 7:00pm CST) Monday-Wednesday or Wednesday-Friday, with quarterly travel to Houston, TX.
What You’ll Do
The Care Team Assistant provides both clinical and clerical support and ensures the provision of quality and compassionate evidence-based care in a virtual Value-Based Care environment. In this position, you will:
- Accept incoming phone calls and return calls from patients involving clinical inquiries for responsible providers, perform protocol-based triage as appropriate, and route phone calls in the form of a phone note document in the EHRS to the provider for review and instructions
- Place calls and create template-based letters, as directed by the provider, regarding all lab results for delivery to the patient in a timely manner
- Review schedule on a daily basis to ensure that all pre-visit preparations have been completed prior to the patient's visit, ensuring lab and imaging results, consultation reports, care transition details, or major changes in health status are available in the patient’s medical record; when these documents are not present in the record, it will be the Medical Assistant's responsibility to call the patient, consultants, imaging centers, and/or hospitals to arrange for timely delivery of said information
- Call patients deemed to be at risk for a “no-show” prior to their appointment as well as new patients establishing care for their onboarding visit to remind them of both the patient’s and Imagines’ responsibilities; contact "no-show" patients and inquire as to their status and need for rescheduling, including assessment of barriers to care and initiation of appropriate social service referrals
- Initiate completion of forms for review and final signature by provider (disability forms, home health orders, durable medical equipment and supply requests, disease status letters)
- Assist care team in regular completion of reports that will allow it to manage and assess the health needs of the patient population; assist care team in recognizing patients who are members of a "vulnerable population" and provide appropriate social service or community-based referrals

As Director of Business Development, you will own and drive deal strategy and execution across defined opportunities and serve as a key thought partner and force multiplier within the Business Development team.
- Develop and drive execution of BD strategy and growth planning (go-to-market strategy, emerging trends and expansion opportunities, partnership development, contract execution)

What You’ll Do
As a Medical Billing & Coding Specialist, you’ll serve in a hybrid role that blends coding precision with billing strategy to ensure timely and accurate claims submission,
compliance, and payment. You’ll collaborate with providers, the data team, and partner operations to streamline workflows, support documentation improvements, and reduce denials.
Billing & Claims Execution
- Submit clean, timely claims with accurate CPT, HCPCS, ICD-10 codes, and modifiers.
- Track and resolve denials, rejections, and underpayments with appropriate follow-up and resubmission.
- Validate eligibility, authorization, and proper billing pathways for all patient encounters.
- Ensure accurate use of telehealth, SDOH, and preventive care codes.
