About the Role:
We’re looking for a builder - someone who loves solving hard operational problems with code, data, and product intuition. You are right for this role if you’re someone who can take an operational workflow, inspect the data, understand the business logic, build and modify tools, evaluate outcomes and ship measurable improvements. Part consultant, part product thinker, part data analyst, you’ll help us automate and scale traditionally manual workflows that underpin healthcare operations - from claim submissions to payment posting.
This role sits at the intersection of product, data, and implementation. You’ll design, test, and operationalize automations across our stack, work hands-on with Product, Engineering teams, and our customers to map and rebuild their workflows, and create systems that scale. You’ll be accountable for outcomes - building the infrastructure, tools, and processes that let our teams and our clients move faster and smarter.

What You'll Do
- Own high-impact processes: Own one or more automation surfaces end-to-end: instrumentation, workflow mapping, SQL/data analysis, tool changes, experiment evaluation, rollout, monitoring, and post-launch debugging.
- Design and automate workflows: Use SQL, APIs, scripts, and emerging tools (including LLMs and harnesses) to automate manual claim-related workflows and improve scalability
- Collaborate cross-functionally: Work closely with Product, Engineering, and Account Management to influence roadmap priorities and scale successful solutions
- Analyze and optimize: Build dashboards and reporting pipelines to monitor productivity, quality, and financial outcomes; identify bottlenecks and automate fixes
What You Bring
- 2+ years of experience in operations, consulting, or data-heavy roles at a tech company or fast-paced startup
- Strong SQL fluency - comfortable querying, analyzing, and structuring data to drive insights and automation
- Excellent problem-solving and systems-thinking skills; able to diagnose root causes and design scalable solutions
- Proven ability to manage complex projects end-to-end, working autonomously in ambiguous, fast-changing environments

About the Role
We’re building a next-generation ambient AI platform for healthcare, helping clinicians focus on patient care by automatically capturing, understanding, and structuring clinical conversations.
Behind that experience is a distributed system responsible for ingesting long-form audio, processing real-time and asynchronous streams, orchestrating multiple AI models, and reliably producing clinical outputs. These workflows must remain observable and recoverable across network interruptions, partial failures, model timeouts, and rapidly changing inference infrastructure.
We’re hiring a Backend Engineer to help build and operate this foundation. You’ll work across audio ingestion, media processing, inference orchestration, distributed workflows, storage, observability, and cloud infrastructure.
This role is ideal for an engineer who enjoys making complex systems dependable. You think carefully about failure modes, idempotency, backpressure, data integrity, latency, and operational simplicity. You also understand that infrastructure exists to serve the product: the systems you build must ultimately create a fast, reliable, and trustworthy experience for clinicians.
What You’ll Do
- Design, build, and operate backend systems that power our Ambient AI products across mobile and web.
- Own critical parts of the audio processing lifecycle, including:
- Real-time and asynchronous audio ingestion
- Streaming and chunked uploads
- Audio validation, encoding, normalization, and processing
- Durable storage and media lifecycle management
- Recovery from interrupted, duplicated, delayed, or out-of-order uploads
- Build and evolve inference workflows that coordinate transcription, diarization, language models, clinical extraction, summarization, and other AI capabilities.
- Develop orchestration systems for long-running, multi-stage workflows, including:
- Scheduling, queueing, and workload prioritization
- Retries, timeouts, fallbacks, and dead-letter handling
- Idempotency, replay, and safe workflow recovery
- Model routing, versioning, and configuration
- Progress tracking and user-visible workflow state
- Graceful degradation when dependencies are unavailable
- Improve the reliability and scalability of services operating under variable, compute-intensive workloads.
- Define and maintain service-level objectives for critical workflows, including availability, processing latency, completion rates, and data durability.
- Build strong observability across services and pipelines through structured logging, metrics, distributed tracing, dashboards, alerting, and diagnostic tooling.
- Lead incident response and post-incident analysis for important production failures, turning operational lessons into lasting improvements.
- Identify systemic failure patterns and eliminate them through better abstractions, automation, testing, and architecture.
- Improve cloud infrastructure, deployment systems, capacity planning, and operational tooling as the platform scales.
- Design APIs and data models that allow mobile, web, and internal systems to interact with long-running workflows safely and predictably.
- Build tools that make inference pipelines easier to inspect, test, replay, evaluate, and debug.
- Balance reliability, latency, quality, and infrastructure cost across AI and media-processing workloads.
- Partner closely with mobile, product, AI, security, and clinical teams to turn new capabilities into production-ready workflows.
- Mentor other engineers and help establish strong practices for distributed systems, observability, operational readiness, and backend architecture.
What You Have
- 4+ years of professional backend or infrastructure engineering experience.
- Experience designing, building, and operating production distributed systems.
- Strong proficiency in at least one modern backend programming language.
- Experience with asynchronous processing, message queues, event-driven systems, or durable workflow execution.
- A strong understanding of distributed-systems concepts such as idempotency, consistency, concurrency, backpressure, retries, failure isolation, and eventual completion.
- Experience operating services in a cloud environment, including deployment, monitoring, scaling, and incident response.
- Experience designing APIs, service boundaries, and data models for complex product workflows.
- A track record of improving system reliability, observability, scalability, or operational efficiency.
- Strong debugging skills across application, infrastructure, data, and external dependency boundaries.
- An ability to reason about both real-time and long-running workloads with different latency and durability requirements.
- Comfort working across backend, infrastructure, product, and AI systems rather than within a narrowly defined layer.
- An ownership mindset and bias for action. You identify important risks, create clarity, and drive ambiguous infrastructure work through completion.

- Experience with audio or video ingestion, streaming media, codecs, transcoding, or media-processing pipelines.

About the Role
This is not a conventional scope management role. You'll serve as the decision-maker and process architect for every request that falls outside standard product boundaries, fielding escalations from Sales, Operations, and Account Management, evaluating feasibility and business fit, and driving fast, defensible resolutions. You'll sit at the intersection of product, operations, and customer service, holding the line on scope while remaining a trusted partner to the business and building the systems that make both possible at scale.
You'll own EHR Credentialing operations across a team of four, mapping workflows end-to-end, eliminating manual and error-prone steps through automation, and tracking the metrics that matter: cycle time, error rate, backlog size, and automation coverage. In parallel, you'll provide the operational backbone for Business Operations, owning onboarding, offboarding, and documentation management workflows to ensure ongoing compliance.
Across all of it, you'll be the process architect codifying exceptions triage, credentialing, and BizOps workflows into documented playbooks, leveraging AI tools and internal platforms like Retool and Vercel to eliminate manual work, and establishing the team's operating cadence through weekly reviews, SLA reports, and cross-functional syncs.
Required
- 5–8 years in operations, product operations, or technical operations
- ideally in high-growth SaaS or healthcare technology with demonstrated experience owning cross-functional processes that required triage, judgment, and stakeholder alignment
- Experience managing a small team, setting priorities, tracking output, and holding people accountable to SLAs while balancing competing commercial and operational needs
- Hands-on experience with SQL and BI or internal tooling platforms (Looker Studio, Retool, Power BI, Tableau, or similar) to build reporting that stakeholders actually use
- Strong process design instincts and you think in systems, document them clearly, and build playbooks that survive personnel changes
- Exceptional written and verbal communication: equally fluent writing policy documents, exception recommendations, and executive summaries
Preferred
- Background in healthcare technology, EHR systems, or clinical workflow operations with familiarity in the credentialing landscape
- Experience in BizOps, GTM Ops, or Revenue Operations supporting Sales or Account Management teams, ideally at the boundary between Product and commercial functions
- Familiarity with AI tools and LLMs for automating repetitive workflows, and enough experience with low-code platforms like Retool or Vercel to collaborate effectively with technical builders
- Obsessively organized but adaptable - you hold complex portfolios without losing the thread when priorities shift, and you manage upward proactively without being asked

ABOUT THE ROLE
At Commure, our Air AI team is building the intelligence layer that lives inside the clinical workflow, that listens, documents, reasons, and acts so clinicians don't have to. You'll work across documentation that transcribes patient visits and structures them into clinical notes in real-time, a conversational clinical assistant grounded in live patient context and medical literature, and an extensible AI Skills Library of reusable agents and workflows that automates clinical tasks across the platform. WHAT YOU'LL DO
- Leverage and build on top of AI/LLM technologies to accelerate product development and deliver intelligent, automated experiences to clinicians and patients
- Design and influence decisions that shape our fullstack architecture for scalable, AI-powered healthcare solutions
- Lead or contribute to technical planning for new features, balancing feasibility, clinical usability, and business impact

ABOUT THE ROLE
Security patterns that worked 20 years ago don't hold up anymore, especially as AI changes how fast engineering teams ship code. We're looking for an Application Security Engineer who combines real engineering depth with security fundamentals — someone who gets into the code, spots systemic patterns, and builds the tooling and fixes that address them at scale, rather than flagging issues for someone else to resolve.
This isn't an audit-from-a-distance role. You'll work directly with engineering, with no interim layer needed, and you'll be equally comfortable owning a project end-to-end as you are looping in support when it counts.
Full Time position requires working 3 days a week in our Mountain View office (Hybrid)
WHAT YOU'LL DO
- Identify systemic security gaps in our codebase and engineering workflows, and drive durable fixes with engineering, not just one-off patches
- Build security tooling and automation, including SAST/SCA integration and custom checks, that catches issues earlier rather than after they ship
- Conduct code reviews and security design reviews for major product initiatives, including agentic AI systems and data pipelines
- Drive threat modeling for new features and translate requirements into guidance engineers actually use
- Think through what AI-accelerated development means for how we find, prioritize, and fix risk, and use AI tooling where it genuinely helps
- Present our security posture to enterprise customers, including healthcare and regulated-data conversations
WHAT YOU HAVE
- 5+ years of hands-on application security or software engineering experience, not mainly audit or compliance work
- Genuine engineering depth: you can read and reason about code well enough to find real bugs and root causes
- Strong AppSec fundamentals: threat modeling, secure code review, and OWASP-aligned vulnerability knowledge
- Comfort operating independently with good judgment in a fast-moving environment
- Clear communication that earns trust with engineers
NICE TO HAVE
- Experience securing agentic AI or LLM-powered systems
- Experience building SAST pipelines or custom static analysis rules
- Healthcare or other regulated-industry security experience (PHI, HIPAA)
- Offensive security background (bug bounty, CTF, pentesting)

About the Role
As a Senior Account Operations Manager at Commure + Athelas, you will play a pivotal role in our Operations team, focusing on Revenue-Cycle Management, by working across the whole organization (Product, Eng, Operations, Sales, and Account Management) to define critical business processes. You will have end-to-end ownership - ensuring efficient execution and a seamless onboarding process for clients and driving business metrics and helping lead the team through the next growth phase. This is an incredibly high-visibility, high-impact, high-autonomy role that will quickly expand to cover more and more emergent business problem areas. Successfully navigating and solving our hyper-growth challenges will lead to rapid growth in role and responsibility.
The position is 100% remote for those NOT in the San Francisco Bay Area, but will be fully on-site for those who are local.
What You'll Do
- Drive client growth through operational optimization and new growth initiatives
- Drive operational metrics (process throughput, defect rate, etc.)

ABOUT THE ROLE
The Commure Ambient team is building the AI-native clinical operating layer for providers. Our AI prepares clinicians before the visit, documents the conversation during it, codes the encounter after, writes structured data back to the EHR, and supports nurses across the full shift. All so clinicians spend more time with patients and less on their screens.
We are looking for a PM who wants to own a product surface end to end and drive deep impact. You'll work with dedicated engineering, design, clinical, ops, and GTM partners, and be accountable for how the whole product succeeds.
WHAT YOU'LL DO
- Set the strategy for a major product surface and drive it from vision through discovery, spec, launch, adoption, and iteration. What you ship this year shapes how care gets delivered across the country.
- Work full stack across the whole product motion. Craft intuitive UIs clinicians actually want to use, set the bar on backend reliability and model quality, and shape the GTM strategy and operational playbooks that drive adoption across clinics and hospitals. You do this alongside dedicated engineering, design, clinical, ops, and GTM partners.
- Sit shoulder to shoulder with providers. Spend real time onsite in clinics, hospitals, and on nursing floors. Watch how care actually gets delivered, feel the friction, and carry that back into every prioritization decision.
- Build direct relationships with clinical and executive customers at some of the largest health systems in the country. Understand their world, ship into their environments, and partner with GTM to drive adoption and expansion.
- Own the metrics that matter. Define the OKRs, use quantitative and qualitative signal to prioritize, and stay accountable to the outcomes.
- Define what best-in-class AI-native healthcare software should feel like. The category is being invented right now, and the products that win will be the ones providers actually love to use.
- Choose your path as the team grows. Build toward managing PMs if you want to become a people leader, or stay hands-on and take on increasingly broad and strategic product surfaces as an IC. Both tracks are real, and both are valued.
WHAT YOU HAVE
We are hiring at both levels and calibrate scope to experience.
- Senior PM: 3+ years of product management experience, with strong generalist fundamentals and a track record of owning products end to end.
- Staff PM: A track record of setting strategy, driving complex product areas across multiple teams, and influencing senior cross-functional stakeholders.
Across both levels, we're looking for:
- Familiarity with AI. You have used and reasoned about LLMs, agents, and evals. You keep up with the frontier and get excited about what's shipping this month, not last year.
- Technical fluency to read documentation, understand APIs and integrations, and hold substantive conversations with engineers.
- A structured approach to user research and a real track record of turning insight into shipped product.
- Sharp written and verbal communication. You can write a PRD that aligns cross-functional teams across engineering, clinical, ops, and sales without a meeting, and hold the room when you do present, whether it's a design review, an exec update, or a customer pitch.
- Curiosity and drive. You are the kind of person who studies the domain, not just the product.
- Comfort with ambiguity and excitement about shaping what your role becomes.
- Consumer product instincts. A sense for building things end users actually love, not just products that clear procurement.
- Genuine passion for healthcare. Clinical experience, time at a healthtech company, or meaningful work alongside providers is a plus, but what matters most is that you care deeply about the space and want to spend your time changing how care gets delivered.

About the Role
At Commure, our Revenue Cycle Management (RCM) AI team is building the autonomous backend that turns healthcare's most manual financial workflows into intelligent systems. You'll ship production AI directly into core revenue cycle operations, with LLM-powered agents and copilots that automate claims submission, remittance posting, denials and appeals, and document extraction with real guardrails and compliance built in. You'll own the full lifecycle of AI features from prompt design and evals through production infrastructure, and partner directly with customers and internal operations teams to turn repetitive manual work into auditable, AI-assisted workflows. We're looking for a Senior Backend Engineer who takes ownership end-to-end, moves fast, and wants their work to directly impact how care is delivered.
What You’ll Do
- Contribute to the technical direction of the RCM platform by proposing and implementing improvements to architecture, tools, and technologies.
