Devoted Health is redefining healthcare. We are seeking a Primary Care Physician (PCP) to lead an innovative clinical model that treats members like family. In this role, you will be a clinician-leader and a builder of the highest-quality virtual primary care model in the nation. Your role will involve deep clinical work managing a shared panel of primary patients, supervising a team of Advanced Practice Providers, developing a suite of industry leading embedded AI-technology, and building out our core clinical model.
You will practice as part of a collaborative interprofessional team while supervising an Advanced Practice Provider pod caring for a panel of patients. You will be supported by industry-leading AI-enabled technology and will be able to refer your patients to a robust suite of internal Devoted Medical specialty clinics.

Responsibilities and Impact will include:
Clinical Care: Deliver primary care within an interdisciplinary team, ensuring effective, patient-centered care. You will be a clinician leader on the Active Care team, responsible for managing diagnostic dilemmas, complex workups, and patients with multiple ongoing chronic disease management needs.
Team Management: Provide supervision, teaching and quality oversight to a team of Advanced Practice Providers. You will foster a collaborative and supportive clinical environment that will help your team thrive. Lead weekly interdisciplinary rounds and readmission conferences to review high-risk patients.
Innovation: Drive innovation and process improvement to enhance the delivery of primary care services. You will be given the tools to rapidly solve problems and develop new ways of delivering primary care with AI and digital engagement as a foundation. The builder work you do will have a lasting and demonstrable impact on our delivery system.
Quality Measurement and Oversight: Our objective is to deliver the highest quality primary care in the nation. You will have the opportunity to build new quality measures, manage quality oversight for your pod, and help us iterate on our balanced scorecard for primary care. Additionally, you will hold collaborative practice agreements with APPs across multiple states.

Required skills and experience:
A board-certified Internal Medicine or Family Medicine physician with demonstrated commitment to primary care
Experience serving diverse patient populations, with a focus on delivering care that aligns with our mission to treat every member like family.
You believe deeply in measuring and improving the quality of primary care delivery across a population.
Desired skills and experience:
Experience supervising Advanced Practice Providers or other healthcare professionals is highly desired, though recent graduates are welcome to apply.
Proven ability to manage change and drive innovation in a dynamic healthcare environment and/or run a clinical program is desired.
Experience with virtual primary care is a plus but not required.
Salary Range: $265,000 - $320,000/ year

A bit about this role:
We’re seeking a highly diligent, process-oriented professional to join our team as a Program Manager. This role is for someone who thrives in fast-moving environments, enjoys building and refining processes, and communicates with clarity. This individual will work across multiple program workstreams at the direction of the Associate Director of the External Audit program. You’ll play a key part in ensuring operational excellence, supporting cross-functional risk adjustment data audit responses, and applying new tools - including large language models - to drive efficiency and quality. This role reports to the Associate Director, Risk Adjustment External Audit Program.
Your Responsibilities and Impact will include:
Support the Associate Director in managing the day-to-day operations of the External Audit Program, ensuring all workstreams are aligned with program goals and regulatory timelines.
Serve as a versatile operational resource, pivoting across workstreams (Retrieval, Evaluation, Submission) to address shifting priorities and volume during the audit cycle
Partner closely with workstream leads to identify opportunities for process improvement and implement changes that enhance scalability, transparency, and quality

Advanced Practice Provider (NP or PA): Palliative Care

Devoted Medical was founded on the belief that if we treat each patient like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. We help members navigate the healthcare system in a better and safer way. This means getting the right care at the right place at the right time.
Our virtual Palliative Care program serves Devoted members with complex needs and life-limiting illness who otherwise would not have access to specialized palliative care. We pride ourselves on providing highly compassionate, patient-centered goals of care counseling, advanced care planning and symptom management for members projected to be in the final year of life. We meet patients where they are and support them in defining quality of life, achieving meaningful goals, and transitioning to hospice care.
Virtual, multi-state palliative care is a novel concept and we are looking for a highly motivated, innovative, experienced palliative care nurse practitioner to help build the foundation of our clinical team. Our ideal NP is an expert palliative clinician with excellent communication and team-building skills who is adaptable to a continuously evolving startup environment.
Skills and experience:
Required:
Must possess a current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state.
Must be willing to obtain additional state licenses as required by the business. (We provide administrative assistance and cover 100% of the associated fees.)
5 years in advanced practice, with at least 3 in Palliative care
ACHPN® credential, or ability/ willingness to obtain within first six months of employment
Willingness to flex between time zones
Comfortable with technology and a fast-paced environment
Self-starter with ability to succeed in a work-from-home environment
Strong interpersonal and communication skills
Comfort working within an interprofessional care team
Preferred:
ELNEC® or EPEC® training
Prior ICU, oncology or geriatrics nursing experience
Prior home-based palliative care experience
Prior interprofessional team leadership experience
Prior program development experience
A Day in the life:

Panel management
Perform initial assessments and develop Palliative Plans of Care for members newly enrolling to the Palliative Care Program.
Follow a longitudinal panel of the highest risk patients (~ 40-50 patients) via regular telephonic and/or video encounters.
Provide consultative visits to patients empaneled to the RNs, including but not limited to: advanced symptom management; complex goals of care; advanced disease education; prognostication; medical orders for home health, DME, medications, and hospice transitions.
Evaluate hospital admissions for root-cause; support transitions of care for engaged patients, identify trends in readmissions and help develop solutions for achieving/ maintaining readmission rate less than or equal to 15%

Host one 30 min office hours per week
Help support the team during periods of rapid growth or PTO/absences; you may be asked to help man the phone line, make outreach calls, perform initial visits, monitor incoming consults, etc.
Collaborate with interdepartmental clinical teams and leadership to help coordinate seamless care of shared patients
