About this role
As the San Bernardino/Orange County area Community Health Worker (CHW) Lead for Waymark, you will lead our community health workers in the region. Reporting to the Market Operations Lead, you will supervise and manage a team of CHWs – including helping them implement their day-to-day priorities, providing coaching and support around patient care, and managing performance to team goals. As a leader in a growing organization, you will also develop and steward community partnerships and resource information to support patient care, and impact how we design, structure, and implement the CHW experience for our employees, spanning recruitment, hiring and training, deployment, mentorship, resourcing, providing pathways to advancement, and advocating for CHWs across our organization and externally.
Key Responsibilities
Provide direct supervision of the day-to-day activities of CHWs in Orange and San Bernardino counties, including helping them establish and implement their schedules and priorities.
Lead weekly team huddles with a multidisciplinary team – including CHWs, licensed clinical social workers, pharmacists, and care coordinators – to coordinate patient care.
Provide CHWs with coaching, mentorship, and support to ensure satisfaction and sustainability within the CHW role, and to ensure competency validation, including by managing one-on-one and group meetings with the CHW team and occasionally shadowing fieldwork.
Manage staffing schedules to ensure patient coverage and continuity of care within the local CHW team.
Review data dashboards to manage CHW performance relative to goals and identify strengths and opportunities.
Work with the Operations and Learning & Development teams to hire, train, deploy, and provide ongoing support to local CHWs.
Navigate technology systems to supervise CHW documentation and ensure quality care
Represent Waymark in the community and build and nurture community partnerships.
Maintain knowledge of programs and resources available to community members that promote their health and wellbeing, and support CHWs in understanding and navigating those resources on behalf of those we serve.
Contribute as a member of the local leadership team by representing Waymark’s values and contributing to a positive and collaborative culture across the local team.
Maintain small outreach and patient caseload responsibilities as a working CHW on an as needed basis to support team coverage, including potentially speaking with patients over the phone or meeting patients in the community, home, or in healthcare facilities.
This is a hybrid role involving work from a home office as well as travel required within the market (up to 60%)
Current Driver’s license and access to an insured vehicle.
Minimum Qualifications
Experience supervising teams of community health workers or related local health workforces.
Experience delivering health services as a CHW or related role and openness to supporting a small caseload
A leader with cultural and personal humility. Has a strong understanding of Medicaid and the barriers to healthcare access and quality among diverse, underserved populations, and is comfortable describing our work externally and building community relationships.
Sound judgment and the ability to quickly analyze situations and manage team members with empathy and care
Experience and comfort using technology for virtual communication, scheduling, and documentation.
Strong understanding of, and ability to teach and implement, principles of trauma-informed care, motivational interviewing, and harm reduction, including among populations facing serious mental illness and substance use challenges.
Ability to travel to local neighborhoods across Orange and San Bernardino counties to help build and nurture community partnerships, establish new CHW teams, and support existing teams.
A person who thrives on building and supervising a team of care providers, building professionalism and positive culture among those working to support the hardest-to-reach patients.
Preferred Qualifications
Knowledge of common CHW recruitment, hiring, training and supervision strategies, and ability to compare and contrast such strategies in real-world situations.
Community Health Worker certification.
Long time resident of the Southern CA area and knowledgeable of community resources.
Hiring Salary Range
$71,000 - $98,000

About this role
As the Outreach Specialist/California Care Coordinator, Manager for Waymark, you will lead our care coordinators in the market. Reporting to the Market Operations Lead, you will have individual contributor responsibilities as a CC as well as management responsibilities for a team of CCs as the market grows – including helping them implement their day-to-day priorities, providing coaching and support around patient interactions, and managing performance to team and individual goals. As a leader in a growing organization, you will also impact how we design, structure, and implement the CC experience for our employees, spanning recruitment, hiring and training, deployment, mentorship, support, resourcing, providing pathways to advancement, and advocating for CCs across our organization.
Key Responsibilities
Provide direct supervision of the day-to-day activities of CCs in Sonoma, Orange & San Bernardino Counties.
Actively participate in the execution of daily CC tasks alongside team members – such as patient outreach, patient care coordination, and provider communication – serving as a role model and contributing to project deliverables.
Lead and support CCs with adequate coaching, mentoring, and support to ensure competency and compliance with workflows.
Work in conjunction with fellow local lead roles and care team members to ensure the seamless execution of cross-team workflows and projects. The overall care team is multidisciplinary and also includes licensed clinical social workers, pharmacists, and community health workers.
Review data dashboards to manage CC performance relative to goals and identify strengths and opportunities to ensure goals are met or exceeded.
Work with the Operations and Learning & Development teams to hire, train, deploy, and provide ongoing support to local CCs.
Foster PCP partnerships through communications and either in-person or virtual meetings.
Continuously build community partnerships through either in-person or virtual meetings at community events.
Maintain knowledge of programs and resources available to community members that may enhance the health care of those individuals.
Foster an inclusive team environment, encouraging open communication, creativity and knowledge sharing. Contribute as a member of the local leadership team by representing Waymark’s values and contributing to a positive and collaborative culture across the local team.
This role is mainly remote with some in-person travel required within the market (up to 30%)
Minimum Qualifications
Deep commitment to improving healthcare for underserved populations.History of supervising teams of care coordinators, patient navigators, customer service, outreach specialists or similar roles.
A leader with cultural and personal humility. Has a strong understanding of Medicaid/MCOs and the barriers to healthcare access and quality among diverse, underserved populations.
A keen eye for detail and proven track record of effectively monitoring and optimizing metrics, leveraging Excel skills or other tools to analyze data and drive continuous improvement initiatives.
Demonstrated ability to collaborate and communicate effectively in a multidisciplinary team setting.
Experience with healthcare data and/or Electronic Health Records.
A person who thrives on building and supervising a team of care providers, building professionalism and positive culture among those working to support the hardest-to-reach patients.
Ability to travel across your local county to help establish new teams and support existing teams.
Must reside in one of these counties: Sonoma/Orange/San Bernardino.
Preferred Qualifications
Bachelor’s degree in health administration, health management, public health, business administration, or related field.
Long time resident of one of the 3 identified counties and knowledgeable of MCO and community resources.
Multilingual skills a plus.

About this Role

About this role
At Waymark, software engineers build the products that enable care delivery teams — community health workers, social workers, pharmacists, and care coordinators — to support people with Medicaid benefits.
As a Junior Software Engineer, you'll be paired with a senior engineer who will mentor you as you build real features and fixes from day one. You'll write code, participate in code reviews, and learn how our systems fit together. We want someone who is curious, asks hard questions, and brings fresh perspectives — we'll invest in your growth as you contribute to meaningful work.
This is a role for someone early in their engineering career who wants to do work that matters.
No prior Elixir experience required — we'll teach it.
What you'll do in your first six months
Ship features and fixes to our messaging platform, built in Elixir
Work through bug tickets across our stack (React, TypeScript, Elixir)
Set up a local development environment that mirrors production
Participate in sprint planning and refinement, managing your work in Jira
Contribute to code reviews on GitHub and keep your team informed in standups
Collaborate with senior engineers, product, and design on upcoming feature planning
Work with PostgreSQL as part of our production data layer
Qualifications
Some experience building software — through professional work, internships, bootcamps, or substantial personal projects
Familiarity with at least one modern web application framework (e.g. React, Next.js, Phoenix, or similar)

About this role
As a Community Health Worker, you will be Waymark’s frontline presence in the community, connecting with patients to provide social support, advocate for their medical needs, and help them navigate health needs.. You will work to improve the health of patients from low- income communities, by connecting them with various providers and resources. You will be part of a care team that is multidisciplinary and includes licensed clinical therapists, pharmacists, and care coordinators embedded in primary care offices.
Key Responsibilities
Attend a virtual 2-week-long paid training program.
Meet patients in the community, in the home, and in healthcare facilities (including hospitals) and conduct a needs assessment, including motivating patients to set and achieve health goals.
Support patients in addressing social challenges such as housing instability, behavioral health concerns, and food insecurity.
Advocate for their healthcare needs and assist them in navigating and accessing necessary medical services and resources.
Work with other CHWs and staff to create a directory of community resources (e.g. food banks, housing assistance programs, childcare resources, etc.).
Manage relationships with the healthcare facility providers and partner with Waymark’s care delivery team (Pharmacists, Therapists, and Care Coordinators).
Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.
Participate in weekly Waymark care team huddles and partner with the Waymark care delivery team (Pharmacists, Licensed Clinical therapists, and Community Health Workers) to support coordinated patient care.
Accurately document each patient encounter in detail.
Minimum Qualifications

About this Role

Work with other CHWs and staff to create a directory of community resources (e.g., food banks, housing assistance programs, childcare resources, etc.).
Manage relationships with the healthcare facility providers and partner with care delivery team (Pharmacists, Social workers, and Care Coordinators).
Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.
Accompany members to medical appointments as appropriate.
Navigate technology systems to document each patient encounter in detail and accurately.
Meet patients virtually, by phone or video visit, for conversations as appropriate.

This is a senior to principal-level individual contributor role who combines deep backend engineering fundamentals with specialized expertise in Electronic Health Record (EHR) data integration. You will report to data engineering leadership and setting the technical direction for our clinical data platform by leading the design, development, and optimization of data pipelines that ingest, normalize, and transform clinical data from diverse EHR and payer systems.

EHR & Partner Integrations
Architect production-grade data pipelines that integrate clinical data through multiple channels—direct EHR connections (e.g., Epic, Cerner, Athenahealth), health information exchanges (HIEs), health alliance networks, and third-party integration vendors—via FHIR R4 APIs, HL7v2 feeds, CCDA documents, and bulk data exports, while enforcing healthcare standards and clinical terminologies (ICD-10, SNOMED CT, LOINC, RxNorm), targeting sub-hour latency from clinical event to actionable insight and ≥99.9% pipeline reliability
Lead end-to-end integration efforts with health plan partners, HIEs, health alliance networks, provider organizations, and data integration vendors—owning partner technical evaluation, connectivity, data mapping, validation, and production rollout
Data Platform & Tooling
Build and optimize cloud-native data infrastructure (AWS) and ETL/ELT workflows using modern orchestration tools (e.g., Step Functions, dbt) with robust data quality monitoring and lineage tracking
Develop reusable backend frameworks, libraries, and internal tooling that accelerate onboarding of new data sources—whether direct EHR feeds, HIE connections, or vendor integrations—with the goal of reducing new-partner integration time by 50% or more while improving developer productivity and reducing operational toil across engineering
ML/AI Enablement & Product Collaboration
Partner with data science and data analytics teams to build and operationalize the data foundations for predictive risk scores, care gap identification, clinical alerting, and patient outreach prioritization
Collaborate with product and clinical teams to translate care delivery requirements into scalable, production-grade technical solutions and influence.
Security & Compliance
Ensure all data systems comply with HIPAA, HITECH, and applicable privacy regulations, implementing access controls, audit logging, encryption, and de-identification processes for PHI
Leadership & Mentorship
Provide technical leadership and mentorship to engineers across Waymark, raising the bar on healthcare data best practices.
Qualifications
Must-have
Significant software engineering experience with substantial focus on healthcare data engineering or EHR integration (Senior: 5+ years; Principal: 8+ years).
Expert-level proficiency in Python (required) and experience in at least one compiled or concurrent language (such as Elixir)
Deep understanding of distributed systems, cloud computing, and backend system design: API architecture (REST), microservices, event-driven systems, message queues (RabbitMQ), caching strategies, and distributed systems patterns
Strong background in data modeling, schema design, performance optimization, and both relational (PostgreSQL) and columnar/analytical databases (Redshift)
Experience with containerization and orchestration technologies (e.g., Docker (required), Kubernetes)
Design data integration systems resilient to inconsistent, incomplete, and delayed clinical data across heterogeneous EHRs and payer systems either directly or through HIEs, health alliance networks, or integration vendors, using FHIR APIs, HL7v2 interfaces, or CCDA parsing

About this Role
As a Community Health Worker, you will be Waymark’s frontline presence in the community, connecting with patients to provide social support, advocacy and navigation. You will work to improve the health of patients from low- income communities, by connecting them with various providers and resources. You will be part of a care team that is multidisciplinary and includes licensed clinical social workers, pharmacists, and care coordinators.
Key Responsibilities
Attend a 2-3 week long paid training program.
Meet patients in the community, in the home, and in healthcare facilities (including hospitals) and conduct a needs assessment, including motivating patients to set and achieve health goals.
Help patients with health-related social issues like homelessness, substance use and hunger.
Work with other CHWs and staff to create a directory of community resources (e.g., food banks, housing assistance programs, childcare resources, etc.).
Manage relationships with the healthcare facility providers and partner with care delivery team (Pharmacists, Social workers, and Care Coordinators).
Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.
Accompany members to medical appointments as appropriate.
Navigate technology systems to document each patient encounter in detail and accurately.
Meet patients virtually, by phone or video visit, for conversations as appropriate.
Support outreach to individuals eligible for Waymark support both in-person and by phone to establish a relationship and let them know about Waymark’s services.
Participate in weekly care team huddles.
Minimum Qualifications
Highly organized and self-motivated to work independently and manage schedules efficiently.
Sound judgment and the ability to quickly analyze situations.
Ability to work with a diverse community in an empathetic, passionate and professional manner.
Friendly, energetic, and enthusiastic personality.
Desire to help others.
Cultural competency- able to work with diverse groups of community members.
Excellent interpersonal communication skills and active listening abilities.
Computer literate with experience and comfort using technology for virtual communication, scheduling, and documentation.
Comfortable with ambiguity and taking on a variety of tasks as needed.
Reside within a commutable distance of Cook County, Illinois.
Must be available to work a set schedule that includes at least one weekend day per week and one late-start shift per week.
Travel required within the surrounding counties (up to 80%).
Current Driver’s license and access to an insured vehicle.
Preferred Qualifications
Community Health Worker certification.
Long time resident of the Cook County area and knowledgeable of community resources.
Experience conducting home visits and outreach.
Experience working with managed care patients.
Experience in customer- or client-service roles
Knowledge of Greater Cook County Medicaid populations.
Hiring Range
$23.08/hr - $26.20/hr

About this role
As an Outreach Specialist/Care Coordinator in the region, you will be Waymark’s frontline presence in and alongside healthcare facilities, often our first point of contact with our patients, and a key member of a startup team helping to shape and refine our workflows and build strong relationships with primary care physician partners. You will work to improve the health of patients from low- income communities by fostering trust and connecting them with various providers and resources. Responsibilities
Initiate outreach to new patients and facilitate connections to healthcare resources and support referrals to care team members telephonically, via SMS, via video, or in-person.
Foster trust and improve patient engagement rates through effective, empathetic, and credible patient communication.
Collect, review and analyze patient data from Electronic Health Records (EHRs) or other data sources to assist monitoring and prioritization of patient care and to improve utilization of preventative care.
Communicate with primary care providers through multiple avenues (potentially including EHR messaging and in-person meetings with providers to review patient updates).
Serve as a primary day-to-day contact, and build trusted, collaborative relationships with healthcare facility providers.
Participate in weekly Waymark care team huddles and partner with the full care team (Pharmacists, Licensed Clinical Social Workers, and Community Health Workers) to support coordinated patient care.
Accurately document each patient encounter in detail.
Collaborate with the Waymark support team to test and refine internal workflows and co-design more efficient processes for supporting patient care.

Minimum Qualifications
Excellent verbal and written communication skills.
Comfort tailoring communication to credibly and concisely communicate with primary care physicians and to communicate with patients in an empathetic and accessible manner.
Highly organized and self-motivated to work independently and manage schedules efficiently.
Customer service and/or sales experience.
Experience with Electronic Health Records.
Experience in a healthcare delivery setting, preferably communicating with medical providers.
Passion for working with diverse communities in an empathetic, inclusive, and professional manner.
Friendly, energetic, and enthusiastic personality with a client service orientation.
Comfort navigating a variety of technology platforms including Google Workspace.
